What is glaucoma?

 

Glaucoma is a group of diseases that are characterised by damage to the nerve that links the eyes and the brain. It is called the silent thief of sight because it attacks first the outside vision, which is difficult to notice in the early stages. Untreated, it will eventually get to the central vision and produce blindness. Once damage is done vision cannot be recovered.

SLT laser
 
This is a mild laser treatment to the  drainage membrane of the eye. The laser has been shown to be as good as drops and long term studies have shown that progression of glaucoma is slower when compare with drops even when ther pressures are similar.
 
This procedure has recently been advised by NICE to be the first line for treatment of primary open angle glaucoma. 
 
SLT gives you the opportunity of having well controlled glaucoma without having to use drops on a regular basis and give you more freedom with peace of mind.

MIGS - iStent W inject
The iStent is a minimally invasive glaucoma surgery that works by inserting two very small stents through the drainage membrane of the eye, therefore allowing clear passage of fluid out. It has been shown in literature to reduce eye pressure and need for glaucoma drops. 
Mr Peixoto has used them extensively and can tell you what you could expect. 
The iStent is indicated for patients with mild/moderate glaucoma. Mr Peixoto will discuss with you if this implant is the most appropriate for you.

MIGS -
OMNI canaloplasty

Canaloplasty is a treatment by which the main drainage canal is flushed with a jelly like fluid to clean it out and to widen it, so that it can drain more fluid and therefore, lower the eye pressure.
The OMNI system is a revolutionary system that makes this procedure super simple and quick. This procedure does not leave an implant behind and it can potentially be repeated in the future.

 videos have been adopted to show surgeries all over the world. 
Mr Peixoto will be able to discuss if this is the best treatment for you. 

MIGS -
OMNI trabeculotomy

Trabeculotomy is a procedure in which the drainage membrane is peeled away so that the fluid has direct access to the drainage that takes fluid out of the eye. 
The OMNI system is a revolutionary system that makes this procedure quicker, safer and leaving less bleeding than other systems. This procedure does not leave an implant behind and it can potentially be repeated in the future. 
OMNI trabeculotomy is indicated for patients with high eye pressures and multiple medications. It is more invasive than other MIGS and therefore it is reserved for patients with more advanced disease

Preserflo microshunt
The Preserflo microshunt is a new device for glaucoma when patients need a low pressure. It works by inserting a very small tube that drains the fluid of the eye outwards into a membrane called conjunctiva, forming a bleb - a blister like structure hidden under your eyelid.
The Preserflo microshunt is indicated for patients with any kind of glaucoma severity but it is a more invasive procedure. Therefore, it is only indicated in those patients who have maximal medical therapy. It also requires more follow up than MIGS (istent or canaloplasty) and requires more frequent post operative medications (which will eventually be stopped).
